  • 2 visa application form filled back to back
  • A covering letter attaching the following documents:
    1. Two duly completed visa application forms with two recent passport size photographs of
    applicant.
    2. Valid passport (at least 6 months validity). Passport data pages should be photocopied.
    3. Evidence of funds available to applicant for the duration of visit. Statement should be
    stamped and signed by the Bank.
    4. A Letter from Place of Employment (stamped and signed by Employer).
    5. A Letter of Invitation and Certificate of Registration from the Guyanese Company.
    6. Flight Itinerary.
    7. Yellow Fever Vaccination Certificate.
    8. Hotel Reservation.
